How to File an Appeal

If your claim for reconstruction was denied, you can file an appeal(s) to your insurance company. You have several options depending on when your claim was denied (pre or post treatment) and the urgency of the appeal. Advocate for Code Reinstatement

Contact your insurance agent, write a letter to an executive at your insurance company, or file a complaint with your state's department of insurance. You can access a letter template to send to your insurance company below.
